Details

Cutivate have an active constituent named Fluticasone Propionate which is a highly selective agonist at the glucocorticoid receptor. Cutivate produces vasoconstriction and anti-inflammatory effects. Fluticasone propionate is used to treat nasal polyps, Crohn’s disease and ulcerative colitis. Fluticasone ointment like other corticosteroid may act by induction of phospholipase A2 inhibitory proteins. These proteins control the biosynthesis of prostaglandins which are potent mediators of inflammation and leukotrienes. Fluticasone on binding with glucocorticoid receptor results in decrease release of leukocyte acid hydrolases and inhibition of histamine and kinin release.

Dosage

Cutivate is an ointment applied to the effected skin twice daily and is available in dosage of 10gm tube and 20 gm. tube. In case of Atopic dermatitis apply the cream twice daily and rub it gently. The dosage range for children and geriatric was unchanged and was applied twice daily due to safety factors being not altered.

How to apply

Before using the ointment manufacturer leaflet present in ointment pack will give you information about steroid and list of side effects observed. Next apply on the inflamed area of the skin and rub it gently until it disappears. The amount of Cutivate applied could vary as per fingertip unit. It is considered that one fingertip unit will cover area twice the size of an adult hand. After applying Fluticasone hands should be washed thoroughly.

Missed dosage - If you miss a dose of Fluticasone propionate cream ply it as soon as possible. If it’s time for the next dose the individual should skip the dose and should follow the regular schedule. Two doses at once are not advised.

Overdosage - Overdosage of Cutivate may lead to symptoms of Cushing Syndrome. Cushing syndrome includes symptoms such as high blood pressure, insomnia, obese trunk, fractures and infections.

Side Effects

There are many side effects which are observed during Cutivate treatment which include burning, peeling, dryness, skin rashes or irritation around the mouth. Redness around the hair follicles, blister, pimples or crusting of treated skin and stretch marks are some other side effects. The changes in colour of skin, thinning and softening of skin are seen side effects.

Contraindication

Cutivate is contraindicated in case of patient who is sensitive to Fluticasone propionate and other component of Cutivate.

Precaution

If the patient is allergic to any component of Cutivate or shows hypersensitivity to fluticasone propionate precaution is needed before use and it needs to be consulted with doctors. Evidences of cushing syndrome, hyperglycaemia and glycosuria could be reported in some of the patients by systemic absorption of topical corticosteroids. Patients who are applying Cutivate on large surface area should be evaluated periodically for HPA suppression. During pregnancy the medication should be avoided and should be discussed with doctors. It is not known if the drug passes the breast milk so kindly refer your doctor. Cutivate is not supposed to be used in case of pre-existing skin atrophy. If skin infection is already present an antibacterial or antifungal agent should be used. The medication should not be used on the face, under the arms and groin areas until directed by physician. The medication should not be applied on the broken skin. Fluticasone use for psoriasis should be consulted with doctor. If the patient is using two topical corticosteroids at one time you should be aware where to apply and consult your physician.

Drug Interaction

Cutivate along with Ritonavir is a potent inhibitor of cytochrome P450 enzymes and leads to high concentration of fluticasone in the blood. In case the patient is using any other prescription or herbal product like Itraconazole and ketoconazole. They are inhibitor of cytochrome P450
